To book an appointment, please call the clinic directly to set up an appointment with your family doctor.
To prepare for your upcoming appointment, please ensure you have a valid health card prior to visiting the clinic.
For in-person appointments, please arrive 5 to 10 minutes early.

All prescription refills require an appointment with your family doctor.
There are no automatic renewals.

If you need to reschedule or cancel your appointment, please provide a minimum of 24 hours notice. Failure to provide the required notice will result in a $40 administrative fee.

Please call the clinic directly if you are requesting your own medical records. Alternatively, you may ask third parties to send us a signed authorization form to request your medical records on your behalf. There will be a charge depending on the amount of administrative and physician time the request requires. This can take up to 10 business days.

The schedule of fees for uninsured services is recommended by the Ontario Medical Association (OMA). For your convenience, services not covered by OHIP can be paid for by e-transfer or cash.
Examples of services not covered by OHIP:
travel advice and travel immunization
missed/cancelled appointments
transfer of medical records
drivers’ medical
employment physicals
children’s camp physicals
removal of some moles/cysts
completion of third party forms
doctor’s notes
